Facelift Surgery


Facelift surgery is a procedure that reduces the appearance of sagging skin on the face as well as the neck. A facelift can make you look five to 10 years younger, depending on skin texture, bone structure, and heredity. Performing treatment on only parts of the ageing face can produce a disharmonious appearance, giving an unnatural or "lifted" look. All aspects of the face, including the brows, eyelids, chin, and neck, are assessed in order to maintain facial harmony.

Modern techniques reshape the underlying layer of connective tissue, called the SMAS, unlike the facelift procedures of the 1970s and 1980s, which stretched only the wrinkled surface of the skin. The manipulation of the SMAS – the direction in which it is moved, the amount it is trimmed or folded and the way it is fixed in place – defines the style of each surgeon. Our surgeons most commonly utilise the Lateral SMAS-ectomy or L-lift, which produces a natural, harmonious appearance.

The Facelift Procedure

The facelift incisions are made within the hairline or within normal lines and creases in front of and behind the ear to minimise visible scarring. Any resulting scars can be concealed by the hair and with the use of makeup. Some facelift variations result in no visible scarring because the incisions are placed within the hair or inside the mouth. Most facelift procedures are performed under general anaesthesia. However, depending on the extent of the procedure and the patient’s request, local anaesthesia combined with a sedative can be administered by an experienced anaesthetist.

Recovery from Facelift Surgery

You should be up and about in a day or two, but plan on taking it easy for the first week after surgery. Most people are back at work about two weeks after surgery. Many patients report a sense of tightness or numbness of the face and neck for a few days after surgery. The intensity of these feelings and how long they last varies according to the individual and the extent of the procedure.


Neck Lift Surgery

In most facelift procedures, the neck, jaw, and jowls are part of the surgery. However, some patients want only their necks to be treated. A neck lift is designed to improve the appearance of loose skin on the neck (what is commonly known as "turkey neck") as well as the common problem of platysma bands (vertical muscle bands in the neck).

The Neck Lift Procedure

Incisions for the neck lift procedure are made in the crease under the chin and behind the ear. The skin is lifted away from the underlying muscles and, if necessary, any extra fat is removed using liposuction. The muscles are usually joined in the midline. It is also possible to join the muscles in the midline and put a stitch through from ear to ear, depending on how tight the patient wants their neck to be. This surgery is most commonly performed under general anaesthesia as a day procedure. Local anaesthesia combined with a sedative, administered by an experienced anaesthetist, may be selected. For younger people who require less work, the procedure may be performed in the office.

Recovery from Neck Lift Surgery

You will be required to wear a chin strap for a week after neck lift surgery and drains are removed after three to four days. Swelling and bruising is minimal and any post-procedural pain can be relieved with medication. Stitches can be removed after a week and you can usually return to work after 10 days.

Is there an ideal age for facelift surgery?
Although it is an individual choice, the 40s and 50s are an ideal time for facial surgery. This is because the facial tissues still have reasonable elastic tone and the extent of the surgery required for rejuvenation may be limited. Facelift surgery performed slightly earlier rather than slightly later will produce a better, more natural and longer-lasting result. Women can have facelift surgery from as young as 35 years to much older. Dr Nettle has performed a facelift on an 84-year-old woman, who sailed smoothly through the surgery and achieved an excellent result.

Does a facelift hurt?
No. You will be surprised at how little pain or discomfort is associated with facelift surgery. A feeling of "tightness" is universal, at least initially, and, depending on the extent of neck muscle tightening, usually lasts only three to four days.

How long does a facelift last?
The improvement achieved in properly performed facelift surgery is to all extents and purposes "permanent". For instance, if you are 50 and a facelift helps you look 43, then when you are 60 you should look 53, and when you are 70 you should look 63, and so on.

How do you avoid the "facelifted" look?
In order to avoid a tight, "lifted" look, choose your surgeon well. The skill and care of your surgeon are a reflection of their training and understanding of this field. Simple techniques to avoid the "facelifted" look are standard practice for surgeons specifically trained in facial surgery. It is important to choose a qualified surgeon who you have confidence in and trust, and to discuss any concerns before the procedure.

Does a full facelift have to be done or are there more minor options?
Many people are content with the gentle improvements possible with skin collagen replacement, dermal fillers and stimulants. However neckline improvement, jowl improvement, and general sagging of the face require surgery. Facial surgery can readily be customised to deal specifically with the areas of most concern – hence the increase in the options available. Mini-facelift procedures and small-incision forehead lifts are frequently performed at our practice, as there is a strong demand for quick recovery and less invasive surgery.

How many facelift procedures can I have?
Facial surgery is usually a one-time event, although about 30 percent of people will have another one after 10 years or so. The first facelift sets the scene for all others. If the incisions have been well chosen, the hairline has not been significantly altered and facial expression has not been changed by indiscriminate SMAS surgery, there is no real limit to the number of facelift procedures that can be done and still achieve a natural look.

Who will notice my facelift?
You may be surprised at how few people notice, especially among your acquaintances. The reality is that most of us (about 80 percent) see what we expect to see, and unless we are expecting it, facelift surgery generally passes relatively unnoticed apart from comments such as 'You look well', 'Have you been on a holiday?' or 'You look very relaxed'.
